• Engage and support essential community providers — adding resources and service capabilities that extend and strengthen the quality of care they deliver

• Focus on and prioritize outreach — engaging the 20% of neediest and most vulnerable patients who drive 80% of expenditures

• Integrate and share information via our web-based, HIPAA compliant CareConnection® platform, which merges multiple data streams and creates a consolidated electronic Community Health Record accessible by all practitioners engaged in a member’s care

• Apply the expertise gained by serving millions of people throughout the U.S. and Puerto Rico to bring proven, tailored solutions to every client
