Disease Management

About 30 percent of employers’ healthcare dollars are spent treating the chronic conditions of about five percent of employees. While there are currently no cures for chronic conditions such as asthma, diabetes and heart disease, APS Healthcare’s Healthy Together disease management programs provide a complete array of services designed to help members live healthier lives and lower healthcare spending.

APS’ Healthy Together programs offer targeted at-risk and condition/disease management programs which focus on the whole person and are designed to help members manage their chronic conditions more effectively.

Because the key to successful disease management is behavior change, behavior modification methods serve as the foundation of APS’Healthy Together programs. APS’ Healthy Together programs have been proven to facilitate healthier lifestyles through changing behaviors of individuals. As a result, APS has a strong track record of positive return on investment for its customers and industry-leading results improving clinical indicators for its members.

APS’ team of multi-disciplinary clinicians work to affect optimal outcomes for individuals with at-risk behaviors, singular disease states and co-morbid conditions. This team builds rapport with members and helps them understand and overcome the obstacles that are impeding them from living the healthiest lifestyle possible.

APS has also developed an innovative community-based disease management model directed at improving outcomes for chronically ill Medicaid beneficiaries. This model involves extensive local collaboration and the placement of health coaches directly in community health centers where a majority of Medicaid patients receive their healthcare. By placing health coaches directly in the public healthcare delivery system, APS has improved Medicaid beneficiary engagement in its disease management programs, increased compliance with recommended care plans and improved coordination with healthcare providers.

APS was recognized in 2005 by the Disease Management Association of America as having the best government disease management program in the United States for its landmark partnership with the state of Wyoming, where APS has exceeded all financial and clinical performance measures.
