State of Georgia Continues Collaboration with APS Healthcare to Provide Medicaid Disease Management Services

APS Healthcare, one of the country’s leading specialty healthcare companies, has completed contract renewals for the Georgia Enhanced Care (GEC) program and the Georgia Medicaid Management Program (GAMMP).

The GEC program, which began in 2005, provides comprehensive disease management services for more than 48,000 aged, blind and disabled members in north Georgia. GAMMP provides a full range of care management and informatics services to approximately 200,000 Medicaid beneficiaries with chronic conditions.

“This is a wonderful, collaborative partnership with APS,” said Dr. Leon Haley, Chief of Emergency Medicine for the Grady Health System. “Our physicians, staff and patients, work directly with APS health coaches. Through this interaction, the patients receive disease education, medication management and most importantly, alternatives for care when they are discharged from the hospital.”

“This is a unique disease management program,” said Dr. George Rust, Director for the National Center for Primary Care at the Morehouse School of Medicine. “The grass roots approach reaches out to community members and partners with communities to serve the population better.”

The Disease Management Association of America (DMAA) recognized APS twice for its GEC work. In 2007, APS was recognized with the “Outstanding Government Program in the U.S” award and in 2006, the company received a DMAA award for “Outstanding Provider Engagement Initiative.” APS was selected by Health Industry Research Companies in Medicaid disease management programs as among the top companies offering disease management programs in the nation.
